Deadline:All applications and responses thereto must be received by the FBA by 5 pm on Friday, July 15, 2016 to be considered for the Family Business Association Awards for Massachusetts 2016.
Who may submit an application?Applications may be submitted by business owners or any party on their behalf.
Special Recognition AwardsThe following awards will be given in addition to the awards presented in the previously noted categories, and will be selected by the judges from the Family Business Association Awards for Massachusetts 2016 applicants: Community Excellence Award (for philanthropic excellence), Adversity Award (for successfully overcoming a substantially adverse situation), Ed Tarlow Marketing Excellence Award (for unique creativity and marketing).

Who is eligible?Any Massachusetts-based family-owned business in one of the four categories below.

Judges will select Finalists and Winners from each of the below categories. Judging for the awards will be conducted by an independent panel of academic, business and management professionals.
A Small Business has fewer than 25 employees (include full time and part time), and has successfully transitioned from one generation to the next.
A Medium Business has between 25-50 employees (include full time and part time), and has successfully transitioned from one generation to the next.
A Large Business has more than 50 employees (include full time and part time), and has successfully transitioned from one generation to the next.
A First Generation Business has at least one family member who is an active equity owner, has at least one other family member who is employed full time by the business, and has not yet transitioned from one generation to the next.

Please address the following topics (approximately 500 words or less per topic):
Give a brief history of your family business. For example: How was the company started, how has it changed over the years, and how many generations have been involved in the operation, owndership, and management? (Please list all names and related roles)
How is your family connected to the business? For example: What is the family’s commitment to the company, and how are responsibilities divided? How does the family balance work with home life? How are your family’s values and beliefs incorporated into your business practices?
Describe your family’s involvement with and the impact your family business has had on the community. For example: Discuss contributions such as leadership, service, and financial support. Provide the names of the organizations and participating family members.
Identify innovative or distinctive business practices which have helped your family business prosper and grow, where applicable, from one generation to the next. Use specific examples.
Describe the impact and success your family business has had in your industry and in Massachusetts. For example: Provide your company’s growth since inception, in sales, employees, profits, number of locations, etc., (financial data is not required). Outline contributions and advancements within your industry.
In your view, discuss what distinguishes your family business as a selection for the Family Business Association Awards for Massachusetts 2016.
(This question is not applicable to first generation businesses) Please discuss transition from one generation to the next. For example: Has succession from one generation to the next affected the quality of family relationships? Describe the process.Applicants will be evaluated using the information responding to the above questions only. If requested within one year, applications will be returned.
